Social Determinants of Health (SDOH) include the social and physical conditions in which people are born, live and work on a daily basis. These factors account for 50 percent of the modifiable contributors to health outcomes and include education, job status, income, social support, safety and the physical environment. The overall aim of this session is to show how well different measures of SDOH quantify and characterize patient risk status in order to optimize a variety of population health and payment purposes. These researchers will evaluate SDOH data from publicly available census data, individual-level information gathered from direct patient screening, as well as hybrid models.
