Those with ADL and/or cognitive needs and their family caregivers are in great need of support across their acute medical and long-term care custodial requirements. Today many entities are supporting families with integrated care delivery and financing approaches. These approaches, mostly within Medicare and Medicaid programs, may be leveraged for public/private collaborations and reduce the burden on family caregivers and public programs. This session will discuss a variety of such examples and allow the audience to see how similar approaches may support and enhance middle-market solutions. Participants will understand how care coordination and management may improve outcomes when integrated across acute and long-term care needs.
