Renal disease is a very heterogenous medical condition whose end state is dialysis, a recurring cost intensive medical condition. Dialysis cost is one of the major medical cost drivers in USA. Insufficient and improper care management in the early stages of CKD (chronic kidney disease) leads to early incidence of end stage renal disease (ESRD) among CKD patients which not only leads to high medical costs, but also reflects poor health quality.
Identifying the right set of low-risk CKD patients who suddenly become high risk and progress to ESRD rapidly continues to remain a major challenge for kidney care providers. Optimal care to CKD patients based on right segmentation techniques would help improving and managing renal care. Come evaluate the current risk associated with the patients and get targeted recommendations to manage the care, delay the progression of the disease and eventually lead to cost saving.
