Description: Risk adjustment is widely used by public and private payers in value-based contracting and other risk-sharing arrangements aimed at addressing population health. Addressing social determinants of health (SDOH) and improving equitable outcomes is one of the aims of population health. Could SDOH be incorporated into risk adjustment? If so, how and how effectively? What implications might this have for market participants? In this session, Joan Barret, FSA, MAAA, from Axene Health Partners will begin by presenting background and findings from the 2022 SOA research report 'How Does Where you Live Impact Your Health?.' This is a question addressed through deep literature review and quantitative analysis, followed by an exploration of strategic approaches and barriers for payers and providers that wish to address SDOH inequities. Then, Rong Yi, FSA, MAAA, and Jeff Milton-Hall, FSA, MAAA, from Milliman will present their new SOA-sponsored analysis regarding integration of SDOH factors into Medicaid managed care risk adjustment. They will cover risk adjustment design considerations, policy and operational considerations for actuaries and states that wish to integrate SDOH into risk adjustment, simulated variation in financial outcomes for risk-bearing organizations when such integration occurs, and variation in the relationship of SDOH factors and morbidity to modeled risk across programs and populations. At the conclusion of the session, attendees will leave equipped to think critically and act effectively with respect to SDOH in their own work, from measurement and analysis design to actuarial interventions.

Description: Human longevity is one of the great success stories of the past century, and there is growing consensus that there are further gains to come. What will these future gains look like, where will they come from and how likely are they to happen. Furthermore, how do we as actuaries deal with the uncertainty when trying to predict future long-term mortality improvements more than 20 years into the future. This is one of the most important assumptions on the balance sheet that insurers and reinsurers make, so it is important to have a strong perspective on it. Our presentation will give attendees a view into some of the core components of how to assess long-term mortality improvements by looking at the cyclical nature across different countries of life expectancy. It will also deep dive into the different drivers of life expectancy over the past cycle driven largely by improved diagnosis and treatment of cardiovascular disease (CVD) and cessation of smoking, which ended in about 2010. It will then discuss how population mortality improvement has slowed and plateaued in many major countries.

Description: Pandemics are a main driver of losses for both life and non-life (re-)insurance. Until COVID-19, risk assessment mostly relied on a few historic examples, most importantly the Spanish flu. The COVID-19 pandemic has shown that other pathogens (namely the coronaviruses) need to be taken into account. Also, modern societies and governments drastically respond to novel threats and implement measures such as social distancing, mask mandates, and lockdowns. Besides the properties of the pathogen itself, these reactions and the medical progression (namely the development of vaccines) are key influences on the course of a pandemic, and thus also the resulting outcomes. To account for the new insights, join us as we take a look at a stochastic, age-structured compartmental model that is calibrated on recent pandemics with several parameters sets to model different levels of reaction speed and stringency between groups of countries. Other important factors also must be defined in close collaboration with experts.
